Please take your time and read/fill out all forms!
We will want to know more about you than most other doctors you have seen before. Yes, the forms take time to fill out, but the more information the doctors have on you the more accurately they can treat you! So please take your time and fill out every form accurately and completely. This assists the doctors in delivering a higher level of quality health care.

Please mail your completed forms along with any medical records (lab reports, etc.). Mail back in 8.5 x 11" envelope.
The doctors do not want to prescribe unnecessary supplements or order unnecessary lab work, so please be thorough! This will help your doctor know if he/she can help you and get you on the road to recovery in the most cost effective manner.
At your appointment expect to pay for your office visit, HCA interpretation, any necessary lab tests (unless we can arrange for lab to bill your insurance), lab collection/handling fee, and any necessary dispensary items.
